Is Forehead Lift Revision Surgery Possible? A Plastic Surgeon Explains Frequency, Timing, and Methods

Many people who are considering a forehead lift, as well as those who have already undergone the surgery, are curious about whether revision surgery is possible, if it will be effective, and when is the best time to do it.

One of the most frequent questions we receive in the consultation room is, ‘I've had a forehead lift once, can I have the surgery again?’

Many are concerned about severe tissue damage in an area that has already been operated on, or whether the effectiveness of revision surgery will be diminished.

Today, I will explain forehead lift revision surgery, focusing on the most frequently asked questions in actual consultations.

Q1. Is forehead lift revision surgery impossible?

Forehead lift revision surgery is entirely possible.

However, it is crucial to first carefully examine why revision surgery is needed, whether a forehead lift is truly necessary, or if another surgery or procedure might be more suitable.

There are two anatomical limitations to a forehead lift.

The first is the length of the supraorbital nerve.

Lifting beyond the range allowed by the nerve is realistically difficult due to the risk of nerve damage.

The second is the elasticity limit of the skin on the glabella and nasal bridge.

Lifting is only possible within the skin's permissible range, so the decision for revision surgery is made after carefully considering these two limitations.

Q2. What is the maximum number of times a forehead lift revision can be performed?

There is no set number of times for this surgery.

However, if a forehead lift is performed correctly, additional improvements are often achieved through eyelid surgery rather than subsequent revision surgeries.

A forehead lift is not merely a procedure to pull the skin; it's a surgery that adjusts muscle balance.

Once it settles stably, it's uncommon for the eyebrows to sag easily again.

Therefore, choosing the method appropriate for your current condition is far more important than the number of times.

Q3. Will my forehead widen if I undergo a forehead lift revision?

This is a concern for many people.

Once tissue is pulled, it can loosen slightly over time, and if pulled again in that state, there is a possibility that the forehead may stretch a little more than before.

However, this aspect varies greatly depending on an individual's tissue characteristics. In fact, many people do not experience forehead widening even after revision surgery.

During consultation, we can make some predictions by directly stretching the eyebrow and hairline areas to check tissue flexibility.

Q4. Is the effectiveness of forehead lift revision surgery reduced?

Many people worry about the effectiveness of revision surgery due to tissue damage or adhesions.

Generally, about one year after the first surgery, the tissue gains some flexibility, making revision surgery possible.

However, it is important to thoroughly consider whether revision surgery is truly necessary and if it will result in a better outcome that is appropriate for the facial proportions.

Assuming the first surgery was performed correctly, the effectiveness of revision surgery may be somewhat limited compared to the initial procedure.

Therefore, revision surgery is considered when the effect of the first surgery was insufficient or when additional correction is needed.

Regarding concerns about tissue adhesion from revision surgery, the dual-plane (plane shifting) method I perform can be helpful.

This method promotes the formation of softer soft tissue adhesions instead of rigid scar tissue adhesions, which is particularly advantageous in forehead lift revision surgery.

Q5. Is the method for forehead lift revision surgery different from the first surgery?

In revision surgery, there are aspects that require more attention than in the first surgery.

First, it is necessary to check if there is sufficient allowance in the glabella and nasal bridge tissues, and in some cases, a preliminary process of releasing the firm scar tissue formed around the supraorbital nerve may be required.

Even in these situations, the plane shifting method has many advantages in revision surgery because it can physiologically reduce the compression and tension of the supraorbital nerve.

Recent studies, including my own research, have also observed that the plane shifting method shows subtly better results in eyebrow lift volume.

I recently presented this content to plastic surgeons at the Korean Society for Aesthetic Plastic Surgery (KSAPS) symposium on the topic of ‘Paradigm Shift in Endoscopic Forehead Lift Based on Long-Term Results,’ and a related paper has been submitted for publication in the American plastic surgery journal PRS (Plastic and Reconstructive Surgery).

Q6. Who typically decides to undergo forehead lift revision surgery?

In clinical practice, the most common cases are those who had surgery a long time ago and experienced sagging again over time.

In addition, some individuals seek revision consultation in the following cases:

• When eyebrow asymmetry remains after surgery, requiring re-correction.
• When the procerus muscle dissection or retaining ligament release was insufficient, leaving room for additional lifting.

Since the cause and approach differ for each situation, an accurate diagnosis must be made before revision surgery.

This patient had their first surgery at our clinic, was satisfied, and lived well for a long time before returning for revision surgery.

Q7. When is the best time to undergo forehead lift revision surgery?

Typically, revision surgery is possible from about 6 months after the initial surgery.

However, I tend to take a more conservative approach, recommending it after at least 1 year, when the eyebrow and scalp tissues have fully recovered.

Conversely, revision surgery within 1 to 6 months after the initial procedure is not recommended because tissue recovery is insufficient, which can lead to increased bleeding and difficulty in dissection.

Conclusion — Forehead Lift Revision Surgery: Accurate Diagnosis is Key

Forehead lift revision surgery is possible, but it's not necessary in all cases.

It is paramount to comprehensively assess whether your current condition can truly be improved with forehead lift revision surgery, or if eyelid surgery or another method would be more suitable.
